245D Waivered Services

In Minnesota, "245D Services" refer to Home and Community-Based Services (HCBS) These services are for individuals with disabilities and those over age 65, provided through Medicaid waiver programs to support independence and community integration as an alternative to institutional care. We are 245D licensed Provider for both basic and intensive support services and we offer the following services:

• Individualized Home Supports (IHS) With Training: This service option is provided to adults when they need support and training in at least one of the community living service categories.
• Individualized Home Supports (IHS) Without Training: This service option is provided to adults or children when they need support, assistance and supervision in at least one of the community living service categories.
• Respite: Respite is a short-term care service intended to provide a break to an individual’s primary caregiver. Respite may be provided in the individual’s home or the respite provider’s home.
• Homemaker Services: Services ranging from light household cleaning with incidental assistance with home management and activities of daily living. Homemakers may monitor the person's well being while in the home.
